Once you have clicked the "Submit" button, you have done all that you need to do
on your
end. We will send an invoice to the address that you provided.
Note: We will begin sending your company the regional reports as soon
as you fill out and
submit the information below. However, failure to pay the invoice will result in
termination of
your subscription. A sales tax is added to all charges for Texas residents.
